Add issue templates to repo

I think issue templates help make for better issues.

You can have a different template for bugs reports and features, and automatically add headings that help normalize the format of the issues.

When selecting a template, GitHub also lets you point certain types of things to other places that shouldn't be issues.

I think a good issue template should have a few clear headings or checkmarks to help make the issue easier to read and more useful for maintainers — but should be fairly minimal so that it is not a major burden or unintuitive process to create one.

I believe this has been resolved with the recent PRs to add to issue and PR templates 👍
