Double-Connect and Double-Disconnect should be errors

If the application calls the Connect() method on a Manta object that's already connected, it's probably a bug in the application that's currently being hidden, only to rear its head later on. This was masking the true cause of #34 and was confusing to the application developer. Similarly for Disconnect().
