Add section with the following info...

PERMISSION TO FILM AND SHARE YOUR TALK

By checking this box you give your permission for JavaZone to both film and share your talk. Please contact us at program@java.no if you have any questions about this.

Should be visible in cake as well

I have added a notice about this in the form now. Adding a checkbox is a bit more work, don't have time for that now.
