require cluster URL only once per scaffolder form and build derived URLs from that base URL #58

There's no need to ask the user to input the cluster URL multiple times in a single scaffolder form.

For example, this could be consolidated because we can build the Microcks and Apicurio URLs from the base cluster domain.
