Custom questions for improvisation

Improvisation questions are added just like normal questions. To be recognised for improvisation, the question must

  1. have a title prefixed by the string {IMPROV} (with braces, no leading or trailing space)
  2. be placed in a category under the activity

Note that IMPROV questions should not (normally) be added to the quiz activity itself.
