There is a special case of error when composing questionnaires: when we compose a host with two refs REF1, REF2 with REF2 being a copy of REF1.

How to reproduce

Create a base host questionnaire HOTE1 (one sequence, one question)
Create REF1 (also one sequence, one question)
Copy REF1 through the Duplicate button in Pogues, call it REF2
Add in HOTE1 both a reference to REF1 and REF2 💥
